Who is eligible for coverage?
All members in good standing of the Fraternal Order of Police under the age of 70 are eligible to apply for coverage.
Members enrolling in the plan may also elect coverage for their spouse and children. Your spouse must be under the age of 70 to apply. Unmarried children from age 14 days to 19 years (up to 25 if a full-time student) are also eligible for coverage. Dependents may not be insured if they are confined to a medical facility.
A spouse or child who is an active member of the Fraternal Order of Police cannot be insured as a dependent. If both spouses are insured as members under this plan, their children can only be insured as dependents of one spouse.
How do I apply for coverage?
Complete and sign the enrollment form. Fax or mail the completed form along with a voided check (for account verification) to:
